We suggest the following wording:

General Bequest of a stated sum of money:

  “I give to Dartmouth-Hitchcock Health, One Medical Center Drive, HB 7070, Lebanon, NH 03756-0001, a New Hampshire nonprofit corporation, or its successor, Federal Tax Identification Number 26-4812335 [insert here the exact dollar amount].”

Specific Bequest of a certain asset from your estate:

  “I give to Dartmouth-Hitchcock Health, One Medical Center Drive, HB 7070, Lebanon, NH 03756-0001, a New Hampshire nonprofit corporation, or its successor, Federal Tax Identification Number 26-4812335, [insert here a description of the particular property].”
